This form should be completed 3-6 months prior to your trip. A confirmation contract will be completed based on the answers to this Questionnaire.

Student Packet

Spanish Student Packet

Parents should review the Parent Newsletter with their child. All students staying overnight at Trees For Tomorrow are required to have a signed student contract and student permission/medical release form on file prior to the start of programming.


Chaperone Packet

All chaperones are required to have a chaperone contract and an adult medical release/emergency form on file prior to the start of programming. Please note, the WI Department of Health Services and Trees For Tomorrow require 1 adult chaperone for every 10 students.

Look at your Confirmation Contract to see what dorms your group is assigned to. Have your group room assignments done prior to your arrival. Please turn in a copy to Trees For Tomorrow staff upon check-in.
